5/31/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Jahsim upon arrival to the home setting. Upon arrival to the home setting, Jahsim appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C engaged him as well as his mother in a discussion to determine behaviors since the last session. 2. Behavior: Jahsim mother reports he continues to struggle with being able to engage nicely with others. 1.Intervention: QMHP-C than began the session by reflecting on positive behaviors since the previous session and behaviors displayed throughout the day. QMHP-C reviewed his medication for compliance and any concerns. QMHP-C discussed the benefits of developing and maintaining peer relationships. QMHP-C reviewed the difficulty that he has had in the past with being able to maintain positive peer relationships. QMHP-C discussed his history of engagement with negative peer relationships in which he has been involved in bullying, aggressive behavior, and other negative behaviors. 2. Intervention: QMHP-C discussed the positive aspects of him not engaging in those negative peer relationships anymore. QMHP-C reviewed how to move forward with peer relationships, being able to identify positive peers to build relationships. QMHP-C discussed ways that he could attempt to build peer relationships. QMHP-C reviewed ways to continue even when one attempt does not work for building or repairing a peer relationship. QMHP-C reminded him that sometimes individuals will hold things from the past against people and that may affect him with being able to build relationships with everyone. QMHP-C gave him examples of ways he may have treated others as reasons it will make it hard for the other person to build a relationship with him. 1.Response: He was receptive to building his peer relationships and ways to maintain others. He expressed a desire to build new relationships and to be able to develop new relationships, especially with new peers at school. He discussed his history with negative peer relationships and stated that he wanted to be able to engage with positive peers. 2. Response: He discussed ways that he could attempt to build new relationships. He stated that he understood that some people may not want to be friends with him and he is okay with that and he will continue to make other friends.
